as shown on said lists are located within 200 feet of the property to be affected by said

application. Said Notice was given by sending registered or certified mail to the last

known address of the property owner or owners, as shown by the most recent tax list of

the municipality or by handing copy thereof to the said property owner, and no one else.

2. Indicated below or attached are the names and addresses of all owners or

property and when they were served, personally or by mail.
