The following is an aid to help you complete the Financial Statement Requirement.

Your parent or sponsor should have this affidavit notarized. Every Financial Statement

should:

1.) contain the name and signature of the sponsor,

2.) contain the address of the sponsor,

3.) be dated,

4.) include a statement that indicates the sponsor takes responsibility for costs

associated with expenses associated with the student’s studies,

5.) include the name of the student

solemnly and sincerely declare as under:

1) That I am the father of (Student’s Name) who is a citizen of Sri Lanka, currently

living at (student’s full address).

2) I am the sponsor of (Student’s Name) in respect to his stay in the USA for higher

studies. I have the financial ability and resources to meet my sponsorship

commitments and obligations.

3) I promise and undertake to bear all his traveling expenses to and from Sri Lanka

and all expenses related to his studies in the USA.

4) I also promise to ensure that he leaves the USA upon completion of his studies

or at any time he is asked to do so by the USA immigration authorities.

I make this declaration conscientiously believing the same to be true. What is stated

above is true and correct to the best of my knowledge and belief. Subscribed and
