√ First Impressions: (pertains to all - Assisted Living, Skilled Nursing & Alzheimer Facilities)
Does the facility smell and look clean and inviting? Does it feel like an institution or have a homey
atmosphere?
Are friends and family welcomed and encouraged to visit?
Is the interaction between staff and residents warm, respectful?
Are residents engaged in activities or lined up in halls bored or waiting for help? Does it look or feel crowded?
Is the dining room calm and quiet? Are residents being helped? Are they socializing?
How flexible is the menu?
How accessible are outdoor areas and is there ample sun/shade?
Are staff engaging residents in activities or babysitting them with TV?
Can residents choose from a variety of activities? Is there an activities calendar posted?
 Quality of Care Questions:
Are healthy snacks available? Is a water pitcher and cup available in each room?
How quickly does staff respond to requests for help?
Do residents have the same care givers on a daily basis or different ones every day?
What is the care plan for residents who get bed sores? What percent of residents have bedsores?
What is the care plan for incontinence? How quickly are they helped to the bathroom? Percent using catheters?
How are resident behavior problems addressed and resolved?
What happens when a resident falls? In what situations do you notify family members?
 Specific Needs:
Is there an Alzheimer's Unit, and is it separate or integrated? How many beds? Is it a secured unit, and are they
required to be in that unit?
How extensive are Rehab resources? How many Physical Therapists on staff? How often do they work with
residents who need rehab?
Can residents see their own doctor or are they assigned resident doctors?
Administrative Questions:
How often are care plan meetings held for residents with their family members?
How long has administrative staff been in place, and what are the results of the last two inspections?
How frequent is the turnover in nursing and CNA staff? What is the ration of each to residents? Are there
nurses on-site 24/7?
What is the plan for an emergency evacuation, or for sheltering in place?
If you already know you are going to need a Skilled Nursing Facility, especially one that takes Medicaid, start
with these two questions when calling around:
Is there a waiting list to get in? How long? What are the requirements?
How many Medicaid beds does this facility have? Is there a waiting list for these rooms?
