Types of Change Requests
1.1.
Examples of types of Change Request Modifications can include: Modifications – to
scope, plans, goals, deliverables, requirements, dependencies milestones or project
planning artifacts, corrective actions - associated with quality, defects, or project process
and baseline change requests to - schedule or budget.
Change Request Submission Process
1.2.
Section 1: Requestor completes Change Request Form Section 1, and
submits to the Project Manager (PM). The PM assigns one or more SME’s
to analyze the proposed change and records in Change Log.
Sections 2-3: SME, Change Request Evaluators provide analysis and
recommendations.
Section 4: - The PM provides a Change Request recommendation, or
modifies the Request. PM determines if the Change Request is viable for a
sponsor or governing body decision and action.
Section 5: Project Sponsor or other governance body – Approved, Modified
or Denied.
Section 6: Project Managers implements if approved, updates change logs
and project plans, and informs requestor of the decision.
Change Request Approval Process
1.3.
1. APPROVAL: If approved, PM implements the change, updates plans, change
form and logs and communicates the decision and reasons to the requestor.
2. DENIAL: If rejected, the Project Manager updates the Change Form and
Change Log and communicates the decision and reasons to the requestor.
