By written consent, signed by all directors entitled to vote on dissolution, in accordance with Section 108.45 of this
Act.
By written consent, signed by all members entitled to vote on dissolution, in accordance with Section 112.10, board

By the members in accordance with Section 112.15, resolution having been duly adopted and submitted to the mem-
bers. At a meeting of members, not less than the minimum number of votes required by statute and by the Articles
of Incorporation were voted in favor of the dissolution. (See Notes 2 and 3 on back.)
By the members, in accordance with Sections 112.10 and 107.10, a resolution having been duly adopted and sub-
mitted to the members. A consent in writing has been signed by members having not less than the minimum number
of votes required by statute and by the Articles of Incorporation. Members who have not consented in writing have
