mediation agreement.

Child support for the minor children shall be ordered as set forth on the attached: Child

Support Information and Order Attachment-Judicial Council Form (FL-342)

Child support is payable through Department of Child Support Services (see attached notice

from DCSS).

Health insurance coverage shall be maintained and health care expenses paid as set forth on

the attached: Child Support Information and Order Attachment-Judicial Council Form (FL-

342).

5. Regarding spousal support (choose only one):

Each party waives the right to receive spousal support, and requests that the court terminate

jurisdiction to award future spousal support to the other party.

Spousal support is ordered as set forth in the attached Spousal, Partner, or Family Support

Order Attachment-Judicial Council Form (FL-343).
