Our customer service representative will be happy to assist you with the many

financial products and services that we offer. If certain ones interest you, our customer

service representative will ask you to complete appropriate forms and applications. As

part of the application process and as required by federal law, we will provide you with

certain disclosures about your legal rights and we will need to obtain your consent

before we can provide you with certain of our products and services.

This package contains the following forms for your information and protection.

You will need to complete certain of them in order for us to provide the financial

products and services that you may request. Should you have questions about any of

these forms, we encourage you to discuss your concerns with your customer service

representative before proceeding to complete a particular form:

1. General Information about you, which we will use to establish our

customer/business relationship with you, which, in turn, will allow us to

contact you about changes and additions regarding our products and services.

2. Privacy Policy of A+, which explains how we protect your privacy and the

security of your nonpublic personal information. In addition, our Privacy

Policy allows you to opt-out of our ability to share certain of your financial

information with select third parties, which provide products and services that

might be of interest to you.

3. Georgia Retail Installment Contract and ACH/EFT/Debit or Credit Card

Authorization Form, which specifies the terms and understanding of the retail

installment agreement and authorizes us to make electronic transfers from your

checking account or debit/credit card to pay fees and charges owed to us and others.
