In order to borrow a book from my personal collection, you must have the form below completed

and on file with me.

This form allows you to borrow one of my personal copies of a novel we are studying in class.

The novel that you borrow must be returned to me in the same condition in which it was issued

to you. If the novel you are issued is lost, stolen, or damaged, you will be responsible for

replacing it with a new copy. I do not accept money for replacing lost, stolen, or damaged

novels.

Novels will be issued to students based on availability. I have a limited number of novels

available for checkout, and I do not have copies of all the novels that will be studied in class.

Therefore, if at all possible, students should purchase a personal copy of any novels that will be

used in a whole-class study.
