Information & Instructions:
1. For applicants requesting fire flow information from the Commission's hydrant flow test database, please complete PART I & II and mail or fax this form to the Field
Engineering Department (See Contact information below. No fees are required for the information request.
2. Applicants shall complete PART III to have a fire flow test performed. Please note that a check in the amount of $437.00 must accompany each fire flow request
Note: The Manager of Field Engineering will notify each applicant when the hydrant test information is available for pick-up.

Part II: REQUEST FOR FIRE FLOW INFORMATION
Note: The Commission maintains a database containing hydrant flow test data and information obtained from previous tests conducted throughout the water distribution
system. Applicants may use this form to request information on file before requesting a Hydrant flow test.
Please provide all available fire flow information for the hydrant(s) identified on the attached GIS maps and noted below:

Part III: REQUEST FOR HYDRANT FLOW TEST
Note: Each fire fow request form must be accompanied by a narrative letter. The letter shall provide a brief description of the fire flow information requested, and shall be
accompanied by a BW SC GIS map identifying the hydrant(s) to be tested.
