ADDITIONAL INFORMATION & REQUIREMENTS For MV-18A AFFIDAVIT
After five (5) years, the Department of Rev e nue retires microfilm/imaged copies of title applications and
documents used to support the issuance of certificates of title. The MV-18A Affidavit must be submitted
to req uest the correction of information recorded on a Georgia Title when the microfilm/imaged copies
have be en r etir ed and are no longer available to validate the correction.
The MV-18A Affidavit can be submitted to your County Tag Office or the Motor Vehicle Division of
the Department of Revenue. The mailing address for the Motor Vehicle Division is:

The MV-18A Affidavit must be completed and submitted with:
1.
MV-1 title application. Application can be electronically completed and printed, typed, or legibly hand-
printed in blue or black ink. The title application must be signed by the vehicle owner(s).
2.
Original Georgia Title unless the original title has been lost or stolen.
3.
No fee is required when the title is to be issued in the same name(s) as the original title. If there is a
and Title Ad Valorem Tax (TAVT) (if applicable)
transfer of ownership, the $18.00 title application fee
must be paid. If it has been more than 30 days since the date of transfer, all associated penalty fees
must be paid.
4.
Based on the type of correction request, the following additional documents are required:
a)
To correct a VIN, form T-22B must be submitted, which has been completed by a Georgia law
enforcement officer, a County Tag Agent or his or her designated employee; or, a legible pencil
tracing of the VIN plate must be submitted.
b)
To remove a salvage or rebuilt brand/legend, a signed letterhead statement from your insurance
company must be submitted, indicating that the salvage or rebuilt brand was placed on the title in
error and the vehicle is not now or has never been in a salvage or rebuilt condition as defined in
State law or, in the event you cannot obtain a letter from the insurance company, a report of a
Motor Vehicle Division Salvage Inspector. To arrange for any inspection, you must complete form
T-22R. This form, the MV-18A affidavit, title application, title, any other required documentation
and a check or money order for the inspection of $100 payable to the Department of Revenue
must be mailed to: ATTN: Salvage, Dept. of Revenue/Motor Vehicle Division, PO Box 740381,
Atlanta, Georgia 30374-0381.
c)
The MV-18A Affidavit cannot be used to change an odometer reading or to remove an odometer
discrepancy brand/legend. To remove an odometer discrepancy brand/legend or change an
odometer reading, Form T-107 (Odometer Discrepancy Affidavit) must be submitted with an MV-1
title application and title. More than one T-107 Form is required if the discrepancy occurred several
owners back or if a determination cannot be made as to when the discrepancy occurred.
5.
Any other available documents that will support the correction request.
The Department of Revenue, Motor Vehicle Division, and County Tag Agents, reserve the right to deny the
correction of any Georgia Certificate of Title.
