Coverage
This form outlines the minimum information for the transfer of locally grown or novelty feeds only. It is not intended to be used for feeds
purchased from Feed manufacturers and/or Importers (e.g. compound meals, grains, PKE, tapioca, and similar). These merchants should
supply you with a specific declaration for each feed type in their product range.
Responsibilities
It is the responsibility of the feed vendor to provide the completed declaration, using this template or an equivalent alternative
It is the responsibility of the feed buyer to:
ensure that documentation is received for all brought in feed and retain for 4 years.
Store the feed in a manner that avoids degradation or contamination
To use the feed appropriately i.e. feed to dry stock or lactating animals to comply with the requirements in NZCP1 and Dairy
company conditions of supply.
Definitions/Clarifications
Forage feeds
Feeds sourced from New Zealand sources other than the dairy platform or ‘run off block’ not under the control of the
person(s) managing the animals that will receive the feed. Includes conserved pasture or crops such as hay, baleage,
silage, maize, etc.
Novelty feeds
Surplus, downgraded, or otherwise unsaleable food products or by-products primarily intended for human consumption
e.g. biscuits, bread, cereals, pastries, lollies, vegetables, fruit, juices, and sauces.
Feed type
Provide a description of the feed. Try to be as descripting as possible and avoid generic terms such silage or food waste.
Examples include hay, grass silage, apple pomace, biscuits, etc.
Source
This will depend on the type of feed. For forage crops such as grass or maize silage, note the address where the feed was
grown, including paddock/ block details where available. For food processing by-products, record the name and location
of the processing plant, orchard, etc.
Total amount of feed delivered using appropriate measurement units, e.g. bales, bins, tonnes, kgs, etc.
Quantity
Carrier details
Record the name of the carrier. Where bulk delivery trucks are used for a range of products (both feed and non-feed)
there is potential for feed contamination. Where contamination is evident the consignment should be rejected.
Enter the date of delivery. Where several consignments of the same product from the same source are delivered over an

Record the stock class the feed is intended for e.g. lactating animals, dry stock, calves, etc. Note that some feeds may

Feed Suitability
Land use
Where feed is produced on land used for disposal of meat, fish, or other industrial wastes, note the name of the
company producing or spreading the waste. In these cases the feed supplier must provide evidence that the waste is
spread in accordance with an approved management plan, and the waste product has been approved for use by Dairy
Processor receiving the milk. Note Some dairy processors do not allow for the spreading of industrial wastes on pasture
that is used for supplementary feeds.
Feed treatments
Spray/treatment residues are a potential source of milk contamination with agrichemicals.
Feeds must not be fed to lactating animals where forage or fruit/vegetable crops have been treated prior or subsequent
to harvest, prior to the expiry of any withholding period for the treatment(s) or if not used in accordance with label
instructions, the feed must not be fed to lactating animals unless approved by the receiving milk processor

Other
Some feed products can modify milk flavour and functional properties. Fruits and vegetables in particular can
result in milk taints and must be used with care. Some common fodder crops and plants that can taint milk are
contaminants
turnips, chicory, onions, land cress, wild onion, wild garlic, pennyroyal and asparagus. Tainted milk must not be
offered for supply.
