Notes: Form 3D – Order authorising reception and detention in an authorised hospital for
further examination
When to use this form:
Following an examination of a person by a psychiatrist at a place that is not an authorised
hospital, the psychiatrist can make an order that the person be received in an authorised hospital
and detained there to enable a further examination by a psychiatrist at the authorised hospital
(s61).
The making of this order is a Notifiable Event which means, where possible, at least one
personal support person must be notified of the making of this order.
A Form 3B – Continuation of detention can be completed if a medical practitioner or authorised
mental health practitioner is satisfied that the person needs to continue to be detained in order to
enable the person to be taken to the authorised hospital.
A Form 4A – Transport Order can be completed by a psychiatrist if satisfied that no other safe
means of taking the person to the authorised hospital is reasonably available.

Notify at least one personal support person that this
order has been made, as soon as practicable.
 Give the person a copy of this form as soon as practicable.
 File this form on the person’s medical record.
 Ensure that the person has the opportunity and means to
contact
any
personal
support
person,
any
health
professional currently providing treatment to the person and
the Mental Health Advocacy Service as soon as practicable
and at all reasonable times when the person is detained.
