Hourly Temperature Log
GENERAL GUIDELINES
Use this log to monitor temperatures during the transport of vaccine and the temporary storage of vaccine during an off-site clinic.
•
Maintain cold chain at all times during transport and storage.
•
Temperatures must be recorded hourly from a calibrated thermometer from the time the vaccine leaves the approved storage unit until it is safely stored
within the same or another approved storage unit.
•
As frozen vaccines are more fragile than other vaccines, they should only be stored in an approved unit and only transport using a portable

freezer unit that maintains temperatures between -50
C and -15
C.
•
Patient transport of vaccine (e.g., zoster) from pharmacy to clinic for administration is NOT an acceptable transport method of any vaccine.
•
Contact the Immunization Unit for specific policies regarding vaccine transport (or refer to The Vaccine Storage and Handling Toolkit pgs. 91-96).
•
Diluent should travel with its corresponding vaccine (do not freeze diluent).
Fax this log in at the end of the month with your monthly temperature logs.
