As your child prepares for the application process, we as counselors endeavor to present to the colleges the uniqueness and

worthiness of your child’s college admissions candidacy. In our efforts to do so, we would appreciate your help in providing a

more multidimensional picture of your child, including the insights into strengths, weaknesses, areas of challenge and

development, personality traits and goals that only a parent can provide. These questions attempt to elicit helpful responses and

are not intended to invade privacy; as such, please answer each question as you deem appropriate. Skip those you are not

inclined to answer. Please feel free to attach your own sheet of paper if your comments do not fit in the space provided or if you
