Instructions:
You are seeking employment with a U.S. Government agency. You must be a US citizen and
complete this form to continue the hiring process. Please take a few moments to answer the
attached questions related to your suitability for employment.
It is of the utmost importance that you answer the attached questions truthfully, completely,
accurately, in good faith, and to the best of your knowledge and belief. Integrity and honesty are
extremely important. Any intentional false answers or statements may disqualify you from
consideration for employment, or result in the termination of your employment after you begin work.
No further action will be taken on your application until this form is received. Information collected
during the pre-employment screening process outlined above is strictly maintained in accordance
with the Privacy Act of 1974, and will not be discussed during your employment interview.
Disclosure of your Social Security Number (SSN) is voluntary. Your SSN will be used to verify your
identity for access to Government facilities and in Government records. The authority for requesting
your SSN is the National Security Act of 1947, as amended. Incomplete addresses and ZIP Codes
may also slow processing.
Special Note: If you are a male born after December 31, 1959, and are at least 18 years of age, civil
service employment law (5 U.S.C. 3328) requires that you register with the Selective Service
System, before you turn age 26, unless you meet certain exemptions. If you are not registered, with
certain specific exceptions, you are not eligible for federal employment. (More information can be
