Commitment Letter

Passenger _________________________ is in need for medical equipment due to his medical condition.

Thus we request to fly the passenger and his accompanies ______________________________

in a medical flight ____________ on ________________ and declare and undertake as follows:

1. We would like to fly with EL AL, although we are aware that El Al may not be properly equipped for such a flight.

2. The medical equipment brought by us (or for us), complies with the list of equipment that was passed and approved

by El Al company.

We are aware that any equipment that does not match the approved list will not be approved to board the flight.

3. We are responsible for the condition of the equipment brought by us, including the ability to use the equipment by

the electric systems on the plane.

4. We hereby declare that all the equipment brought by us (or for us) is able to function properly throughout the

planned flight, including in case of delay or diversion of the flight path.

5. Following is a list of medical equipment/technical terms in the aircraft which we ask to be provided by El Al (Circle the

relevant equipment):

6. We declare and undertake not to make any claim or lawsuit against El Al (or anyone on her behalf) in the event of

different or additional technical requirements from what is described in this document.

7. We hereby declare that we hold all travel documents and certificates required in the destination country. If additional

documents or certificates will be required, we will be responsible for obtaining and presenting them to the

authorities.

8. We are responsible for reserving and coordinating the arrival of the ambulance in the destination country (if required)

and hereby release El Al from any obligation related to the arrival or it's suitability for the passenger's medical needs.

El Al will not be responsible for transporting the passenger (if required) in and around the airport in the origin and

destination country, or beyond, and that the commitment begins with the departure of the flight and ends with the

arrival of the plane to the gate and the opening of the planes' doors, except for providing assistance in coordinating

the entry of the immigration authorities into the plane.

10. El Al will not be responsible for the refusal of entry to the destination country by the local authorities.
