2015-2016 Binding Commitment Letter

A. Offer Period for Boy’s 15U and older. If I tryout and receive an offer for a Boys 15U or older club team, I

have until 7:00 PM on Sunday, September 27, 2015 to make my official commitment to a club.

B. Offer Period for Girl’s 15U and older. If I tryout and receive an offer for a Girls 15U or older club team, I

have until 7:00 PM on Sunday, November 22, 2015 to make my official commitment to a club.

C. Offer Period for Boy’s/Girl’s 14U and younger. If I tryout and receive an offer for a 14U or younger club

team, I have until 7:00 PM on Sunday, October 25, 2015 to make my official commitment to a club.

In all cases, if a club makes an offer, the offer cannot be rescinded unless I decline the offer; I do not sign a Binding

Commitment Letter accepting the offer by the official commitment date / time listed above; or if I do not respond by

the commitment date/time listed above. For tryouts which occur after that date and time, there is no waiting period

and I may be asked to officially commit to the team immediately.

Response to Offer. When I am notified that I have been selected for a team, I have 3 response options: 1. To

ACCEPT the offer by signing the Binding Commitment Letter; 2. To DECLINE the offer by signing the Binding

Commitment Letter; or 3. To USE THE “WAITING PERIOD” provided in Section 1 above before making my

decision (I do not sign the Binding Commitment Letter). If I have chosen Option #2 or #3 above, I may attend other

tryouts, without losing any offers made to me.

Other Tryouts. I understand that when I accept an offer by signing this Binding Commitment Letter, I am

committed to the club below and that I should not attend any other club tryouts for the season.

Only one (1) Acceptance Binding Letter of Commitment Permitted. A player who accepts a club offer by signing

a Binding Commitment Letter will be affiliated with that club for the entire 2015-2016 season. I understand that if I

back out of this commitment, I forfeit my eligibility to participate in Gateway Region Junior tournaments and USAV

national events with any other club during the 2015-2016 season.

Club Signature Required Prior to Submission. This document must be signed and dated by a club representative

before submission to the player or parents (or legal guardian) for their signatures.

Parent/Guardian Signature Required. My parent or legal guardian is required to sign this Binding Commitment

Letter if I am less than 18 years of age at the time of signing.

Falsification of Binding Commitment Letter. If I falsify any part of this Binding Commitment Letter, including

the date, I understand that I am subject to suspension for the entire 2015-2016 season and/or other penalties.

Binding Agreement. My signature on this Binding Commitment Letter supersedes any agreements, verbal or

otherwise, which would release me from the conditions stated within this Binding Commitment Letter. I understand

that I have signed this Binding Commitment Letter with the club and not with a particular individual. If the coach or

any player(s) leaves the team, I remain bound by this Binding Commitment Letter. The terms of this Binding

Commitment Letter may not be amended without the written approval of the Region. I understand that I may be

released from this Binding Letter of Commitment only with the written approval of an authorized club

representative.

In the event that a team has less than 6 committed players by 8am the Friday immediately following

tryouts, the commitment letter, if signed by myself and my parent/guardian/legal representative, will be voided and I

will be released to find another team.

Commitment to the Club. I agree to pay the club’s disclosed fees for this season and to support the club in word

and deed. I certify that I have read this Binding Commitment Letter carefully, I have discussed any questions

regarding this Binding Commitment Letter with the club representative, and I fully understand, accept and agree to

be bound by this Binding Commitment Letter.

10. Compliance by Club. If the Region determines that the club has violated the Region’s tryout policies with respect

to a player, the club will be required to release that player from his or her Binding Commitment Letter, and such

player then may tryout for or affiliate with another club. The club committing such violation will be subject to

sanctions by the Region, which may include forfeiting the right of their team or teams to participate in Gateway

Region or USAV national events.
