CREDIT REPORT AUTHORIZATION AND RELEASE

Authorization is hereby granted to Crown Mortgage, LLC and its associated lender(s) to obtain a standard factual

data credit report and any other credit report or automated underwriting report deemed necessary by the loan

representative for purposes of my mortgage loan inquiry or mortgage application through a credit reporting

agency chosen by the loan representative and/or lender.

My signature below authorizes the release of a copy of my credit application, and authorizes Crown Mortgage,

LLC and/or its associated lenders(s) to obtain information regarding my employment, asset accounts, and

outstanding credit accounts. Authorization is granted to use a photo static or facsimile reproduction of this

authorization if necessary to obtain any information regarding the above mentioned information.

Applicant(s) hereby requests a copy of the credit report obtained with any possible derogatory information be sent

either by electronic or standard mail to the address listed on the application. The applicant(s) holds Crown

Mortgage, LLC, the loan representative, and any credit reporting organization harmless in providing the

requested copy. The applicant(s) agrees to reimburse Crown Mortgage, LLC for its cost of obtaining the

report(s).
