2Kaitiaki Coordination Service
3D Coordination is a referral and information service
for children with Developmental, Attention or Behaviour Needs
links families/whanau with available/appropriate services
Role: To improve access to services for children with mild to moderate learning, attention
and/or behaviour needs by streamlining the referral process, facilitating community supports
and advocating for services as necessary. 3D Coordination will review the referrals to ensure
that systems are effective and families make progress along the referral pathway.
Criteria for enrolment to 3D Coordination:
The child:
Is 14 years of age or younger.
Is resident in the Western Bay of Plenty (bounded by Waihi Beach in the North,
Otamarakau in the East and lower Kaimai ranges in the South).
Has a developmental, learning or attention difficulty of clinical or educational
significance.
May have associated disruptive behaviours.
Requires a medical check-up and referral from a G.P (G.P referral letter in pack).
The child does not currently access services from:
Child Adolescent and Mental Health Services – CAMHS.
Ministry of Education ORRS.
Ministry of Education, Special Education Behaviour Services.
Who can enrol child into service?
Parents can enrol to get the initial assessment pack with questions to be completed.
Educators, health professionals and community agencies can enrol a child with the parents’
permission and the initial assessment pack will be sent to the family. The family need to get
a GP referral following a medical check-up to complete the referral process.
