STATEMENT OF VOLUNTEER APPLICANT

If offered a volunteer position, I agree to abide by the YMCA Code of Conduct.

I understand there is a 6 month term for volunteer service. Volunteer service is solely “at will,” giving the

volunteer or the YMCA the right to terminate volunteer service at any time without liability or obligation.

I understand that there will be training as a volunteer that I must attend.

I certify that the facts contained in this application are true and complete to the best of my knowledge

and understand that, if accepted as a volunteer, falsified statements on this application shall be grounds

for dismissal. I authorize investigation of all statements contained herein and the references and former

employers listed above to give any and all information concerning my previous employment and any

pertinent information they may have, personal and otherwise, and release all parties from liability for any

damage that may result from furnishing same to you. I understand that if selected, my volunteer term

with the YMCA of Rapid City will be contingent upon of a criminal background check and meeting and

maintaining full compliance with the YMCA of Rapid City criminal background standards.

I hereby acknowledge that I have read and understood the above statement and that I voluntarily sign

this application.
