We believe you should have a certain amount of input into the letters of recommendation that
we write to colleges and universities on your behalf. Consequently, we are asking you for your
perceptions of your educational and personal growth. Be specific. Be anecdotal—tell stories
about what actually happened in a given situation that show who you are. Take your time—the
more information you give, the more detail your counselor can include in your recommendation.
Remember—colleges want to get to know who you are, and the answers to these questions will
help us provide this information.
You are not necessarily limited to the space provided or to the questions below; you may add
relevant information if you like. Feel free to respond electronically; this form is available on the
high school guidance department’s website. If you complete a paper copy, you may attach an
extra sheet.
