SPECIAL GROWLERS LICENSE
We, the undersigned Licensees, request permission to sell refillable containers of draft beer for
off premises consumption.
1.
Check the appropriate box below:
The Applicant License allows the sale of alcohol for off premises consumption.
The Applicant License does not allow the sale of alcohol to be taken off the premises.
AFFIDAVIT BY LICENSEES
The undersigned do solemnly affirm under the penalties of perjury and upon personal knowledge
that the contents of the following paper are true:

I/We are over the age of 18 and competent to testify to the matters stated below.

I/We are the Licensees for a Class ________________________ License “License” on
behalf of ______________________________, located at _____________________________________
(the "Premises"), and certify that the following:

In conjunction with the issuance of the Growlers Special License, the following terms of
Md. Ann. Code, Art. 2B, § 8-202(a) & (b) shall be complied with:
a.
The refillable container used by the Licensee(s) shall:
i.
have a capacity of not less than 32 ounces and not more than 128 ounces
ii.
be sealable;
iii. be branded on the container with an identifying mark of the Licensee(s) (a copy of
which is attached);
iv. bear the federal health warning statement required for containers of alcoholic
beverages which is printed on the container (no stickers);
v.
display instructions for cleaning the container which is printed on the container (no
stickers);
vi. bear a label, which is printed on the container (no stickers), stating that cleaning the
container is the responsibility of the consumer and the contents of the container are
perishable and should be refrigerated immediately and consumed within 48 hours
after purchase; and
vii. A picture of the proposed growler(s) to be used showing compliance of the aforesaid,
with the size of the container marked, is attached.

The hours of sale for a refillable container begin at the same time as those for the current
License but sales must end at midnight.
