volunteers of Puppy’s Playpen, LLC, at their sole discretion, and that I assume full

financial responsibility for any and all expenses involved.

Owner specifically represents that he or she is the sole owner of the pet, free and

clear of all liens and encumbrances.

Owner specifically represents to pet care facility that the pet has not been exposed to

any communicable diseases within the last thirty-day period prior to boarding.

All charges incurred by Owner shall be payable upon pick-up of pet, or at pet care

facility’s choice when billed by Puppy’s Playpen, LLC. At address listed above. The

pet care facility shall have, and is hereby granted, a lien on the pet for any ad all

unpaid charges resulting from boarding, daycare or grooming at Puppy’s Playpen,

LLC. The Owner hereby agrees that in the event the boarding, daycare or grooming

charges are not paid when due in accordance with this contract, Puppy’s Playpen,

LLC. May exercise its lien rights upon three day written notice given by Puppy’s

Playpen, LLC. to Owner by certified mail to address shown on contract. Puppy’s

Playpen, LLC. May dispose of pet for any and all unpaid charges, at private or public

sale, in the sole discretion of the pet care facility, and Owner specifically waives all

statutory or legal rights to the contrary. If such sale shall not secure a price adequate

to pay such costs of board or other delinquent charges, plus cost of sale, then Owner

shall be liable to Puppy’s Playpen, LLC. for the difference. All monies realized by

the pet care facility at such sale, over and above the charges due and cost of sale,

shall be paid by the pet care facility to Owner.

If pet, becomes ill of if the stat of the animal’s health otherwise requires professional

attention, the pet care facility, in its sole discretion, may engage the services of a

veterinarian or administer medicine or give other required attention to the animal and

the expenses therof shall be paid by the Owner.

FEES: If a pass is purchased it is valid for six(6) months from the date of purchase.

VIP passes can also be purchased for a year from time of purchase.

Reservations: Are REQUIRED. Cancellations with less than 24hours notice for

daycare and 48 hours for boarding will be charges a penalty fee of 25.00.

This contract contains the entire agreement between the parties. All terms and

conditions of this Contract shall be binding on the heirs, administrators, personal

representatives and assigns of the Owner and Puppy’s Playpen, LLC.

health and have and have not been ill with any communicable condition in the last 30

days. I further certify that my dog(s) have not harmed or shown aggressive or

threatening behavior toward any person or any other dog. I have read and understand
