CHAPTER 13 TRUSTEE QUESTIONNAIRE & DOCUMENT REQUEST

PART I: INTRODUCTION & INSTRUCTIONS

As a Chapter 13 Debtor, you have the duty to fully cooperate with and assist the Trustee in the

investigation of your financial affairs. The completion of this form and the submission of the required

documents are included in that cooperation. Further, everything you submit in regards to your bankruptcy,

including this document, the documents requested in Part II, and your testimony at the § 341 Meeting of

Creditors, must be true, complete, and accurate. If you have made any errors, you must inform your

Trustee immediately or correct your errors. It is a federal crime to provide false or misleading

information in regards to your bankruptcy. Please refer to the Bankruptcy Code, the Local Bankruptcy

Rules, and your Trustee’s Guidelines for additional details.

PART II: DOCUMENTS

The Attorney Document List, which can be found under the Forms Section at , includes a

list of required documents that must be submitted to the Trustee no later than seven days prior to the

§ 341 Meeting of Creditors. A valid government-issued picture identification and original social security

number verification must be presented to the Trustee at the § 341 Meeting of Creditors.

Debtor, if applicable, must respond to each question. It is a federal crime to present false or misleading

information to the Trustee.
