I am the designated POC for the above-referenced Sprint business account (the "Business Account") accepting ownership of the

phone number(s) or account(s) below and/or contained in the attached document or any other document executed in connection

with or relating to the change of ownership (the "Transferred Number(s)") on behalf of the Business Account. I hereby authorize

the change of ownership of the Transferred Number(s) to the Business Account.

I understand that Sprint may contact the applicable account owner of the Transferred Number(s), and disclose my and my

company’s identify to such account owner, to facilitate the processing of the requisite Change of Ownership Release Form.

I agree that the Business Account must provide this "Release Change of Ownership" link

( ) to the account owner(s) releasing ownership of the Transferred Number(s) so that

such account owner(s) may complete its part of the change of ownership process. The change of ownership process will not be

completed until such account owner(s) completes the applicable requirements.

I understand that as a result of the change of ownership of the Transferred Number(s), the Business Account will accept all

liability, financial and otherwise, for the Transferred Number(s) after the change of ownership becomes effective, including

liability for any remaining installment or lease payments owed pursuant to any Installment or Sprint Business Lease Agreement

that may be assigned to the Business Account in connection with the change of ownership. I further understand that as a result of

the change of ownership, the Business Account's use of the Transferred Number(s) will be governed by the terms and conditions

of the service agreement between the Business Account and Sprint.
