individual account. By signing below, I represent that the information provided in this Change of Ownership Acceptance Form is

accurate and true and that I: (i) am an authorized signatory for the Business Account and have the full right and authority to bind

the Business Account to the terms and conditions contained in this Change of Ownership Accept Form; (ii) authorize the change

of ownership of the Transferred Number(s) to the Business Account; (iii) understand that Sprint may contact the account

owner(s) of the Transferred Number(s) to provide such account owner(s) with information to complete the change of ownership

process; (iv) agree that the Business Account will indemnify, release and hold harmless Sprint from any claim or harm that may

arise out of any contact with such account owner(s); (v) agree that the Business Account is bound by the applicable terms and

conditions of the Business Account's service agreement with Sprint; (vi) agree to accept all liability for the Transferred

Number(s) after the change of ownership becomes effective, including any liability pursuant to any Installment or Sprint

Business Lease Agreement that may be assigned to the Business Account in connection with the change of ownership; and (vii)

on behalf of the Business Account, agree to the terms and conditions set forth in this Change of Ownership Acceptance Form and

any additional documentation required by Sprint in connection with the change of ownership.
