In addition to verbal intervention techniques, staff will be trained and therefore only authorized, to use the techniques specified in
this document. The techniques checked are those that have been identified by the department and must be included in your
training program. Check additional techniques that staff will be trained to use, as it relates to your facility or program.
As you complete the “contact information” section, ensure you provide the name of a person who the Department can contact for
additional information or to provide procedural updates. After developing your plan, obtaining all applicable signatures and dates,
