3. The above named domestic employer hereby elects to report quarterly wages and pay
taxes annually, and agrees to provide wage figures at other times as necessary to
establish wage credits for claims purposes.
4. Quarterly reports and taxes will become due and are required to be reported and paid
by the employer no later than January 31 with respect to wages for employment paid
in the preceding calendar year. The employer will file their quarterly reports on one
combined form which will be mailed to the employer in December of each year.
5. The effective date of this election is the first day of January,
(year) and shall
be for a minimum period of two (2) calendar years, except that the Commission may
terminate this election if the employer becomes liable under another category of
employment. It is understood and agreed that this election must be received no later
than December 31 to be effective at the beginning of the next calendar year. It is also
understood and agreed that this election may be withdrawn by the employer by filing
a written application with the Commission not later than December 31 of the year
prior to the beginning of the year with respect to which the organization wishes to
withdraw.
