Line 3. Give the name of the state or country where it was incorporated.
Line 4. State the present name and address of the registered agent in the State of Hawaii, before any change.
Line 5. State the name of the new registered agent in the State of Hawaii. If there has been no change, state NO CHANGE.
For Profit corporations: The agent must be an individual resident of Hawaii, a Hawaii corporation, or a foreign
corporation authorized to transact business or conduct affairs in the State of Hawaii.
For Nonprofit corporations: The agent must be an individual resident of Hawaii or a Hawaii corporation.
Line 6. State the new address of the corporation’s registered office in the State of Hawaii. Give the number, street, city, state
and zip code. If there has been no change in the address, state NO CHANGE.
