NOTE: Minors can only be taken outside of the student's major curriculum. The Curriculum Committee Approved Minors are listed
below.
STUDENT: 1) Fill in the top part of the form. 2) Take the form to the department offering the Minor, to complete the requirements
section. 3) Take the form to your advisor for his/her approval. 4) Bring the completed form to the Registrar's Office. The yellow
copy will be date stamped and returned to you. Keep this copy so you will know what courses you must take in the future semesters.
MINOR DEPARTMENT: 1) Review requirements for the Minor with the student. 2) Fill in the course information. 3) Check
Column A, B or C: If a course is specifically required for the Minor, check Column A. If "X" number of courses from a particular list
are required, fill in the number of courses required (in the column headings) and indicate the courses in this list by checking Column B
or C.
ADVISOR: Review the student's Minor request to make sure it is not inconsistent with his/her Major Program and sign the form.
