This form is to be used by committees (non-candidates) to register as required by Colorado law.

Yes.

When do I file this form?

This form must be filed before accepting or making any contributions. CRS 1-45-108(3).

COMPLETING THE FORM

Indicate by placing an (x) the type of committee you wish to form in the box next to the corresponding

committee type.

Print or type the full name of the committee.

Print or type the physical address of the committee.

Print or type the mailing address of the committee.

Print or type the phone number of the committee.

Print or type the E-mail of the committee, if you wish your E-mail address to be public record (optional).

Print or type the Web Site of the committee if you wish your Web Site address to be public record

(optional).

Print or type the name of the registered agent.

Print or type all affiliated candidates and committees, if applicable.
