2REQUIREMENTS FOR THE ISSUE OF A SEAMAN’S RECORD BOOK AND CERTIFICATE OF DISCHARGE
The Applicant must:
1. Complete an Application Form and submit to the Registrar of Seafarers, along with:-
a. A valid Passport evidencing Nationality
b. Seaman’s Identity & Nationality Booklet (optional)
c.
Two certified passport-sized photographs taken recently ( six (6) months or less)
d. Police Record issued within the last twenty four (24) months; if copy, it must be certified by a JP.
e. Medical Fitness Certificate on the prescribed form, from a doctor on the MAJ’s approved list of medical
practitioners, valid for not more than two (2) years
f.
A valid Basic Safety Training Certificate (BST).
g. Evidence of qualifications for position being offered on the vessel.
h. The relevant fee.
2) Present either
a. A request for the issue of a Discharge Book from an approved Maritime Training Institution;
OR
b. Letter from an approved crewing agency or shipping lines/ship owner/manager offering employment
and providing the following details:
I)
The full name and Passport Number of Applicant
II) Employment offered
III) Proposed date and port of engagement and discharge
IV) Name, Nationality, Type and Official Number of vessel for proposed assignment:

c.
Discharge Certificate or Letter of Discharge stating:
i)
Applicant’s Name
ii)
Port of Engagement and Discharge
iii)
Capacity of Employment (rating)
iv)
Name, Nationality, Type and Official Number of vessel
