Important Notice:
If applying for an off-premises beer and wine permit, wholesale beer and wine permit, or 7-day beer and wine permit, I do
hereby stipulate that I will not permit the on-premises consumption of beer and wine on the licensed premises. I
understand that a violation of this stipulation will be a violation against the permit and shall constitute sufficient
grounds to suspend or revoke the beer and wine permit.

Notice requirements: In addition to the newspaper advertisement described in the attachment to this application, an
agent of the S.C. State Law Enforcement Division must post a notice at the proposed place of business. This sign must
remain posted for at least fifteen days, and must be removed by the SLED agent. If the SLED agent returns to remove
the sign, and the sign is not posted, the agent must re-post the property. This will delay issuance of your license or
permit. If the sign disappears before it is removed by the SLED Agent, contact the S.C. Department of Revenue
immediately.
Enclosure Checklist:
1.
Completed application, signed, dated and notarized.
2.
Proper filing fees.
3.
Copy of lease, deed or other ownership documentation.
4.
Submit an Affidavit of Publication from the newspaper running your ad. The affidavit must include a copy of your ad.
If you have just started your ad in the newspaper and have not received the ad and affidavit, you must include the
receipt from the newspaper with your application. Forward the ad and affidavit from the newspaper as soon as it is
provided to you by the newspaper. Your license/permit cannot be issued until this is received. (ABL-32)
5.
Schedule B must be completed for all applications.
6.
If you are applying for a restaurant minibottle license, you must submit a current Grade A health inspection.
7.
If you are applying as a nonprofit private club, you must include a copy of your clubs bylaws and a copy of your
Articles of Incorporation.
8.
If you are applying as a corporation, limited liability company, partnership, or a limited partnership and any of your
principals are nonresidents of SC, you must attach a current criminal history background check obtained from the
state of residency for each nonresident. These background checks must accompany your application. See
Schedule B for definition of principal.
9.
If applying under Section IX, 120 days temporary permit or license, attach:
a.
$25 for each temporary permit requested. If applying for a temporary beer/wine and minibottle, $50 would be
required.
b.
Attach a copy of bill of sale, lease, probate document, divorce decree, or eviction order or document showing
how the applicant acquired the business.
c.
If the applicant is not currently licensed at another location, a current criminal history background check is
required on the applicant if applying as sole proprietor. If applying as corporation, a criminal history
background check must be furnished for each principal of a corporation, partnership, limited liability company
or limited partnership. (See Schedule B for explanation of principal.)
d.
Must attach the existing permit/license.
e.
Must attach a current Grade A health inspection. (For restaurant minibottle only)
f.
Must attach a copy of Articles of Incorporation if applying for nonprofit private club.
g.
Must attach a copy of bylaws if applying for nonprofit private club.
