1UCC-11 INSTRUCTIONS
1.
For accurate entry, this document should be typed in UPPER CASE.
2.
Uniform Commercial Code request for information or copies from the Secretary of State will be complete
searches, at a fee of $15 for each business or debtor name. Please note a complete search will be
performed. Copies are $1.25 per page, plus $1 for certification for each file number.
3.
Only those names and addresses of debtors listed will be searched. If a specific address is listed, only that
address will appear on the completed copy search. On information searches, all addresses will be checked.
If no address is listed, all addresses found in our data base will be included on the completed search. The
certification date on the certificate is the most recent date which has been completely processed and appears
in the Uniform Commercial Code records.

There are three options in Colorado for filing liens. The collateral or security used determines where to file:

The state has a prepaid account system which will allow many functions to be handled via faxed requests.
