NOTE: YOU MUST ATTACH ALL SITE SCHEDULES TO THIS RETURN. THE TOTALS ON THE SITE SCHEDULES MUST
REPRESENT THE CURRENT TAX YEAR.

This return must be accompanied by a certificate from an independent C.P.A. attesting to an audit which shows that the amount of Chicago
rental revenue reported is computed in accordance with the requirements o f chapter 3-32 of the Municipal Code of Chicago and that national
rental revenue, fleet size, additions and transfers are accurately reported for the relevant periods. This certificate is an integral part of the return
and if it is not submitted with the return, the return will be deemed incomplete. Late filing penaltiee will be assessed in these cases. See Uniform
Revenue Procedures Ordinance Ruling #1 dated 919196.

Enter the Chicago rental revenue for the period of April 1 of the prior year through March 31 of the current year. CNcago rental
revenue means all revenue derived from rental of automobiles in a rental fleet as computed for the purposes of the Chicago Personal
Property Lease Transaction Tax, Chapter 3-32 of the Municipal Code of Chicago.
Enter the national rental revenue for the pedod of April 1 of the pdor year through March 31 of the current year. National rertal
revenue means all gross consideration for renting of passenger automobiles at all locations in the United States valued in money
whether received in money or otherwise. National rental revenue does not include charges for insurance, fuel, or taxes (see Section
3-56-010 of the Municipal Code of Chicago.
Calculate the Chicago Rental Percentage by dviding line 1 by line 2 (carry 4 decimal places).
Enter the number of automobiles in the national rental fleet as of March 1 of the current year. "Rental fleet" means passenger
automobiles under common ownership and used in an automobile rental business.
Number of vehicles subject to wheel tax license (multiply line 3 by line 4).
Preprinted tax rate.
