The undersigned applicant hereby applies for reservation of the following entity name for a

period of one hundred twenty (120) days:

The name is being reserved for the following type of entity pursuant to the applicable

statutory provision. The appropriate fee is enclosed with this application for the type of

entity specified below:

Corporation (including business, non-profit, professional and foreign

corporations and professional associations) pursuant to Article 2.06

of the Texas Business Corporation Act or Article 2.04A of the Texas

Non-Profit Corporation Act ($40.00).

Limited Liability Company (including foreign limited liability companies)

pursuant to Article 2.04 of the Texas Limited Liability Company Act ($25.00).

Limited Partnership (including foreign limited partnerships) pursuant to

Section 1.04 of the Texas Revised Limited Partnership Act ($50.00).

Name reservations filed under one statute cannot be used for, or transferred to, filings made

under any other statute

In addition, once the application is filed, the name reservation will

be recorded exclusively in the name of the applicant. Transfer of the reservation to another

person may be made by filing a notice of such transfer executed by the applicant for whom

the name was reserved and paying the appropriate fee.
