NAME OF CORPORATION:

Please provide the complete name of the corporation, as it

appears on the records of The Secretary of the State.

APPOINTMENT

OF NEW REGISTERED AGENT:

Please provide: a.) full name of the individual and complete addresses.

b.) complete name of business and address (P.O. box is only acceptable

as additional information).

Note: The corporation

may

appoint itself as its registered agent. The corporation

may appoint either a natural person who is a resident of Connecticut; a Connecticut

business or a foreign business, which has authority to transact business in Connecticut. If

the agent being appointed is a natural person, that person’s business address maybe

provided under the heading Business address. Their residence address must be provided

under the heading Residence address. If the agent appointed is a business, its principal

office address must be provided under the Business address heading. The agent must sign

accepting the appointment and the signatory must state the capacity under which they

sign, if signing on behalf of a business. All addresses must include a street number, street

name, city, state, and postal code. P.O. boxes are unacceptable.

EXECUTION:

The document must be executed by an authorized official of the

corporation. That person must print or type their name, state the capacity under which

they sign and provide a signature. The execution constitutes a legal statement under

the penalties of false statement that the information

provided in the document is

true.
