If you have changed your corporate officers or partners, attach a list showing current officers or partner's

names, title and address.

Your Business Registration Certificate will become permanent upon payment of $30 fee. This certificate will

be valid until cessation of your business or until revoked, suspended or cancelled by the Tax Commissioner. If

you are exempt from the fee, line one (1) you are still required to register. Continuing to operate a business

without obtaining a registration certificate and paying the tax is a crime and may result in fines and/or

imprisonment, W. Va. Code § 11-9.

your "Reissuance of Business Registration Certificate (Permanent)" (WV/BRT801A) and mail in the

envelope provided. The new bond form is available at Failure to comply will delay the
