To Whom It May Concern:

For public utility assessment purposes, 66 Pa. C.S.§ 510(b) requires that every public utility file with the

Commission a statement, under oath, showing its gross intrastate (jurisdictional) operating revenues for the

preceding calendar year. This statement has been formalized for Electric, Gas, Telephone and Water/Wastewater

utilities on the enclosed ASSESSMENT REPORT (Form GAO-09). Revenues reported on this form are for the 2009

calendar year.

Public utilities that fail to file a properly completed, signed and notarized Form GAO-09 by March 31,

2010, will be in violation of 66 Pa. C.S. § 510(b). If any public utility fails to file a timely statement on the form

provided, the Commission will estimate such revenues for determining assessment and the estimate is binding upon

the public utility. While the Commission will proceed to estimate the revenues upon which your company’s

assessment will be based, a formal complaint will also be filed against your company that could result in the

imposition of a fine. Violations of the Public Utility Code are punishable by a fine of up to $1,000 for each day of a

violation pursuant to 66 Pa. C.S. § 3301.

Please read the enclosed instructions carefully before preparing your Assessment Report. The instructions in

bold are changes or additional information requested this year. The completed original report with signature and

notarization must be filed with the Commission’s Assessment Section no later than March 31, 2010. No

extensions will be granted.
