Checklist of Required Materials to Apply for a

Wyoming Certificate

All applications for an initial Wyoming Certificate must include the following fully-completed

materials:

1.______Completed Sections A, B, C, D, E, F, and G. (H is optional)

2.______Official transcripts from the college or university where coursework was completed.

COPIES OR FAXED COPIES ARE NOT ACCEPTABLE.

3.______Section C must have original signature and seal. No copies or faxes are accepted.

4.______Appropriate non-refundable fees. $130 evaluation and $45 fingerprinting (may be one

check.) Fee (personal checks are accepted) must be made payable to the Professional

Teaching Standards Board.

5.______Completed Fingerprint Cards ORANGE AND BLUE.

6.______Applicants with a certificate from another state should include a copy of this valid out-

of-state certificate. Send only certificates that are current.

7.______Applicants verifying experience must include a letter from a supervisor indicating

successful experiences.

8.______The ORANGE Fingerprint Card must be completed, signed, and notarized. Completion

of the reverse side of the this card must be part of the package.

All required data on the application must be complete, including dates and

signatures.

Failure to complete all required items on your application can result in your

application being returned to you without action.
