Please print this form and submit by mail. Include applicable fees in the form of a check or money-
order payable to the City of Nashua, along with a self-addressed stamped return envelope, and a copy
of the rabies certificate and spay/neuter certificate, if applicable.

FOR A PUPPY:
$7.50 (4 to 7 months old)
UNALTERED MALE OR FEMALE DOG:
$10.00 (NOT spayed/neutered)
SPAYED/NEUTERED DOG:

SENIOR CITIZEN RATE:
$3.00 (fee applies to first dog only)
GROUP LICENSE:
$20.00 (5 or more dogs)
FINES:
$1.00 per month is assessed starting June 1
of each year and on the first day of each month

thereafter until the dog is licensed. In addition, annually at the end of June a warrant for
unlicensed dogs is provided to the Animal Control Officer to issue a civil forfeiture fine of $32.00 for
every unlicensed dog.
