Consent to Obtain Insurance Score*
With your permission the company will obtain your insurance score, and if it is found to be at or above a
predetermined qualifying level you will receive a credit on your policy.
Insurance Score: An insurance score is a numerical ranking of aspects of a person’s credit history. Insurance
companies use the score to rank the probability of loss of groups of insureds and to provide discounts to
customers who have higher scores. Claims data from large numbers of insurance customers show that customers
with higher insurance scores as a group tend to have lower claims losses than customers with low scores.
Vermont privacy laws require that companies must get permission from consumers in order to obtain a credit
report or history. An Insurer cannot check your eligibility for their Credit Score discount without your
permission.

In connection with my personal auto and/or home insurance policy obtained from Paige & Campbell, Inc., I
hereby consent to the insurance company or agent obtaining a credit report or insurance score for me which may
contain information as to my credit standing, credit worthiness, credit record or related information. This
authorization applies to subsequent reports in connection with the renewal or changes to my policy obtained
thru Paige & Campbell, Inc.
