I, the undersigned, certify that the motor vehicle hereinafter described is more than twenty (20) years of age and such motor vehicle has not
been altered or modified from the original manufacturer's specifications except to assure normal running operation or to meet specific safety
requirements on original equipment, or both.
**Part II — Street Rod
I, the undersigned, certify that the motor vehicle hereinafter described was manufactured before 1949 and has been altered or modified and
is referred to as a "Street Rod".

INSTRUCTIONS
1.
This form is to be completed by applicant and filed with title application when registering a special interest vehicle. Part I is for motor
vehicles more than 20 years of age; Part II is for vehicles manufactured before 1949, which have been altered or modified and are
referred to as "Street Rods".
2.
Special Interest vehicles, including street rod vehicles, may be used as are other vehicles of the same type, except that special interest
vehicles, including street rod vehicles, may not transport passengers for hire, nor haul material weighing more than 500 pounds.
3.
A collector must own and have registered one or more vehicles with regular license plates which are used for regular transportation.
