Enter the City. (There is a 22 character maximum).
Enter the State Abbreviation. (e.g. NC).
Enter the Zip Code. (e.g. 27604).
Enter the Zip Code Extension if applicable. (e.g. 7299).
Save the file.
5. The following information must be entered to create the Employee Information: (Note: Each employee
record should be on a different line and all information should be separated by using a comma. Enter the
first employee record on the second line underneath the employer record. See Figure A below)
Enter the Social Security Number. (Do not use hyphens. e.g. 546238844).
Enter the First Name. (There is a 15 character maximum).
Enter the Middle Name or Initial. (There is a 15 character maximum).
Enter the Last Name. (There is a 20 character maximum).
Enter the State Employer Account Number. (This is a 9 digit North Carolina Employer ID).
Enter the State Taxable Wages. (Do not enter a decimal to separate the dollars and cents. E.g.
123300 will be understood to be $1,233.00).
Enter the State Income Tax Withheld. (Do not enter a decimal to separate the dollars and cents. e.g.
123300 will be understood to be $1,233.00).
Save the file.

6. Wages and withholding data must be included in the employee information. Where dollar amounts are
zero, enter zeros. Blanks are not a valid entry. (E.g. $14.00 should be entered as 1400, not just 14.)
7. Burn the file to a CD-ROM and mail to:

8. Multiple payers/companies may be filed on a single CD-ROM. A separate Form NC-3 Annual
Withholding Reconciliation must be included for each business entity.
9. The filing deadline for submission on CD-ROM is April 1, 2013.
10. If the CD-ROM is unable to be processed, the contact person will be contacted to resubmit the data.
11. While it is necessary to sign up to use electronic media for Social Security Administration reporting, it
is not necessary for NC reporting.
