FOR THE REPORTING PERIOD SUBJECT TO TAX
This report may be filed in summary by customer.
Column 1
Enter the name and address of the customer receiving the moist snuff.
Column 2
Enter in ounces the weight of the moist snuff being reported. Total this column and enter the total on Line 11 of the TPT-20.
NOTE: THIS SCHEDULE DOES NOT INCLUDE SALES AT RETAIL UNLESS THE TAX WAS NOT PREVIOUSLY CHARGED.
