Worksheet #2: Earned Income Only Tax Base
Use this worksheet to determine your liability for school district income tax due to an earned income only tax base
school district as shown in the list on page 3.
If you fi led a school district income tax return for year 2010 and if you will be making estimates based on the “last
year’s tax” method, skip lines 1 through 5 and enter on line 6 below 100% of the tax shown on line 7 of the 2010
Ohio form SD 100.
Note 1: If you move during the year and you will be residing in more than one school district that assesses the school district
income tax, then you will need to complete a separate worksheet for each school district.
Note 2: You can avoid paying school district estimated income tax if the sum of (i) your 2011 Ohio income tax withholding
and (ii) your 2011 school district income tax withholding and (iii) $500 is equal to or greater than the sum of your 2011 Ohio
income tax and your 2011 school district income tax.
1. Enter on line 1 your earned income you will earn while a resident of an earned income only
school district listed on page 3. “Earned income” means wages (see line 7 of IRS form 1040
or line 1 of IRS form 1040EZ or line 7 of IRS form 1040A), tips, other employee compensation
shown on your W-2 and self-employment income that you will report on IRS Schedule SE (part

4. Senior citizen credit. Enter the senior citizen credit of $50 if you (or your spouse, if fi ling jointly)

payment credited to year 2011. Exclude from your year 2010 overpayment any tax paid after

Credit Card Payments
You can use your Discover/Novus, VISA, Master Card or American Express card to pay your estimated Ohio income taxes.
Make your credit card payments either by calling 1-800-2PAY-TAX or by visiting tax.ohio.gov and clicking on the “Make
a Payment” link and selecting the Ohio ePayment option. There is a convenience fee equal to 2.5% (or $1, whichever is
greater) for this service. This fee is paid directly to Offi cial Payments Corporation and is based on the amount of your tax
payment. Do not fi le Ohio voucher SD 100ES if you use the credit card method of payment.
Federal Privacy Act Notice
Because we require you to provide us with a Social Security number, the Federal Pri-
vacy Act of 1974 requires us to inform you that providing us with your Social Security
number is mandatory. Ohio Revised Code sections 5703.05, 5703.057 and 5747.08
authorize us to request this information. We need your Social Security number in order
to administer this tax.
