GENERAL INSTRUCTIONS

 PHA-F Membership is open for general public (I-16/3, Islamabad), including

all Pakistani citizens, Azad Kashmir and Gilgit Baltistan, and overseas

Pakistanis of 18 years of age or above having valid NADRA CNIC are

eligible for membership.

 Membership fee is Rs.10000/-.(Non- refundable) and valid for only I-16/3,

Project, Islamabad

 Membership form along with demand draft shall be submitted in head office

of PHA Foundation, Islamabad.

 Please type or print your particulars in the form in CAPITAL letters.

 All entries in the forms must be duly filled in English.

 Cutting / overwriting or ambiguous entries shall not be accepted.

 Latest photograph shall be affixed on form where specified duly attested by

gazetted officer of Government of Pakistan.

 Attested copy of CNIC shall be enclosed as well as pasted on the form, duly

attested by gazetted officer of Government of Pakistan.

 In case of overseas Pakistanis attested copy of Passport shall be enclosed,

duly attested by gazetted officer of Government of Pakistan.

 Signature on application should match with Computerized National Identity

Card (CNIC).

 Demand Drafts of Rs. 10, 000/- (Non-refundable) in favor of PHA

Foundation, Islamabad, should be submitted along with membership form,

requisite down payment and documentation charges of Rs. 15,000/-.
