c) was unable to produce or was not issued with an original NPF/NSITF membership certificate

for submission to facilitate the transfer as required by the Guidelines on Transfer of NSITF

Contribution to Employee RSA.

d)My former employer which made the contributions on my behalf is no longer in existence.

Now therefore, I Mr/Mrs/Miss do solemnly declare, undertake and become legally bound as

follows:

1. That I shall indemnify National Pension Commission/Trustfund Pensions Plc for any loss

or claim that may arise as a result of the transfer of NSITF contribution into my

Retirement Savings Account.
