c. Clearance from the Bureau of Immigration

d. Work Permit from the Department of Labor and

Employment, if applicable

* All photocopies shall be verified against originals.

6. President’s Sworn Statement that:

 the corporation will not accept or solicit investments and/or

incur loans from more than nineteen (19) lenders including

shareholders;

 in case of unauthorized acceptance or solicitation of

investments or borrowings, it shall immediately indemnify or

return the investments of persons upon presentation of valid

claims;

 in case there is/are foreign stockholder/s, the country or

state of a foreign stockholder/s allows Filipino citizen/s and

or corporation/s to do lending business therein;

 the company shall comply with the Rule 17.1(2)(c) of the

Amended Rules and Regulations of the Securities

Regulation Code.
