5 Attach a copy of a notarized statement of a first-time home buyer
A first-time home buyer is an individual who has not owned and occupied a residential complex in Canada as a primary residence
in the past 60 months. If the new home is to replace your primary residence that was recently accidentally destroyed, you may also
qualify as a first-time home buyer. Attach a copy of a statement witnessed by a notary public that verifies you are a first-time home buyer
.
6 Who will use the newly constructed home as a primary residence?
An owner or co-owner
A relation of an owner or co-owner (Attach a notarized statement)
If the new home is for a relation—someone related to you by blood, marriage, common law relationship, or adoption — attach
a copy of a statement witnessed by a notary public that explains your relationship to this person. The relation must also qualify as
a first-time home buyer.
7 Sign the certification and consent
I certify that the information given in this application is correct to the best of my knowledge.
I certify that I, a co-owner, or a relation of one of us will use this home as a primary residence once completed, not as an
investment property or recreational cottage.
I consent to the use of my personal information for the purpose of determining my eligibility for the rebate program.
I understand that my personal information will be kept private under the terms of the Freedom of Information and
Protection of Privacy Act.
I understand that I am required to keep all original documents associated with this rebate for 6 years in case I am audited.
